ELI5: What Are Nursing Diagnoses?

A nursing diagnosis is a statement about what health problem the patient has (or is at risk for). It's different from a medical diagnosis - it's what the NURSE can treat!

Formula: Diagnosis (Problem) + Related to (Cause) + Defining characteristics (Signs/Symptoms)

🔝 Most Common NCLEX Diagnoses

1. Anxiety

Related to: Stress, threat to self-concept, change in health status

Signs: Restlessness, trembling, tachycardia, verbal expressions of worry

2. Risk for Infection

Related to: Invasive procedures, immunosuppression, inadequate nutrition

Prevention: Handwashing, aseptic technique, monitoring WBC

3. Acute Pain

Related to: Tissue damage, injury, surgical intervention

Signs: Guarding, grimacing, vital sign changes, vocalizations

4. Risk for Falls

Related to: Age, impaired mobility, medication side effects

Interventions: Bed alarms, non-slip footwear, frequent rounding

5. Impaired Physical Mobility

Related to: Musculoskeletal disorders, pain, weakness

Signs: Limited ROM, inability to ambulate, bedridden

6. Ineffective Airway Clearance

Related to: Obstruction, secretions, aspiration

Signs: Cough, adventitious breath sounds, dyspnea

7. Risk for Impaired Skin Integrity

Related to: Immobility, poor nutrition, moisture

Prevention: Turn every 2 hours, pressure-relieving devices

8. Deficient Knowledge

Related to: Lack of exposure, information misinterpretation

Intervention: Teach using simple language, provide written materials

9. Ineffective Coping

Related to: Inadequate support, situational crisis

Signs: Verbalization of inability to cope, substance use

10. Impaired Gas Exchange

Related to: Alveolar-capillary changes, airway obstruction

Signs: Confusion, cyanosis, abnormal ABGs
