Learn to tackle multiple choice, SATA, and more!

ELI5: Question Formats

Think of the NCLEX like a video game with different types of enemies. Each type needs a different strategy to defeat! Some questions are straightforward, while others require more thinking.

📝 Multiple Choice (Most Common!)

The classic format - you'll see this the most (about 70-80% of questions).

Example: "A client with diabetes is experiencing hypoglycemia. Which intervention should the nurse implement first?"
A) Administer insulin
B) Provide orange juice
C) Check blood glucose
D) Notify the physician
Strategy: Read the question carefully - look for keywords like "first," "best," "most important." Eliminate obviously wrong answers first.

☑️ Select All That Apply (SATA)

These have multiple correct answers - could be 2-6 options. You must select ALL correct answers, not just some.

Example: "Which of the following are signs of hypoglycemia? Select all that apply."
A) Sweating
B) Polyuria
C) Confusion
D) Hypertension
E) Shaking
Strategy: Treat each option as true/false. The answer could be 1, 2, 3, or ALL options. Don't select an option just because it's partially correct.

📋 Ordered Response (Put in Order)

You'll arrange items in correct sequence - like prioritizing nursing actions.

Example: "Place the following nursing interventions in the correct order:"
A) Assess the client's pain level
B) Administer pain medication
C) Evaluate medication effectiveness
D) Document the intervention
Strategy: Use the nursing process (ADPIE): Assess → Diagnose → Plan → Implement → Evaluate. Or ABCs: Airway → Breathing → Circulation.

📊 Fill-in-the-Blank (Calculations)

Usually math problems - dosage calculations, IV rates, conversions.

Example: "The physician orders 500 mg of medication. The medication is available in 250 mg tablets. How many tablets should the nurse administer?"
(Answer: 2)
Strategy: Know your formulas cold! Practice, practice, practice. Write down the numbers before calculating.

🎯 Hot Spot

You'll identify a specific area on an image or diagram.

Example: "Click on the area where you would listen for the mitral valve sounds."
Strategy: Know anatomy! Be familiar with heart sounds locations, landmark positions, and wound locations.

📝 Chart/Exhibit

You'll review medical records, lab values, or care plans before answering.

Example: "Review the client's lab results, then answer: Which finding requires immediate intervention?"
Strategy: Don't skip the exhibit! The answer is usually in the chart. Focus on abnormal values.
