ELI5: What Are All These Tests?
Prenatal testing is like getting a sneak peek at how the baby's developing. Some tests screen for potential issues, some diagnose them. It's information gathering - not every "risk" becomes a reality!
TL;DR: Screening tests tell you RISKS, diagnostic tests give DEFINITE answers. You don't have to do any of them!
๐ฌ Screening Tests (No Risk)
These tests estimate risk but don't give definite answers:
First Trimester Screen (10-13 weeks)
- Blood test: Measures pregnancy-associated plasma protein (PAPP-A) and hCG
- Nuchal translucency ultrasound: Measures fluid at back of baby's neck
- Combined with age, gives risk for Down syndrome and other trisomies
Cell-Free DNA Test (NIPT) - 10+ weeks
- Blood test analyzing baby's DNA in mom's blood
- Screens for Down syndrome, trisomy 18, trisomy 13
- Can also tell you baby's sex!
- More accurate than first trimester screen (99% for Down syndrome)
- Now available for twin pregnancies too
๐ฉบ Diagnostic Tests (Some Risk)
These give definitive answers but carry small risks:
Chorionic Villus Sampling (CVS) - 10-13 weeks
- Takes sample of placental tissue
- Diagnoses chromosomal conditions (like Down syndrome)
- 1 in 300-500 risk of miscarriage
Amniocentesis - 15-20 weeks
- Takes sample of amniotic fluid
- Diagnoses chromosomal conditions, neural tube defects
- 1 in 300-500 risk of miscarriage
- Can tell you baby's sex too
๐ Understanding Test Results
Based on data from Emily Oster's research:
The key question: What's the difference between "high risk" and actually HAVING the condition?
Example with Down syndrome:
- At age 25: 1 in 1,250 chance of having a baby with Down syndrome
- If "high risk" first trimester screen: 1 in 100
- But 99 out of 100 "high risk" babies are actually fine!

Questions to ask yourself:
- Would knowing change anything about this pregnancy?
- Would I consider termination for a genetic condition?
- Do I want time to prepare for a child with special needs?
- Am I comfortable with the small risk of diagnostic tests?
- What would make this worth it for me personally?
There's no right answer! Different families make different choices based on their values and circumstances.
